                       CASE HISTORY
Patient details:
A 13 years old female resident of kommattigudam came to hospital. 
Cheif complaints:
    Shortness of breath with exertion since 1 yr. 
    Easy fatigability since 1 yr. 
History of present  illness :
Patient was apparently asymptomatic 1 year back. 
Since 1 yr she started feeling difficulty in breathing on running for small distances and also while playing running games with her friends that is due to breathlessness and easy fatigue which is insidious onset and  gradually progressive and relieves at rest. 
Then recently she attended a regular medical examination conducted in school, that advised her to visit hospital. 
History of past illness:
No fever, cough, cold, pain. 
No diabetes mellitus, Asthma, TB, or any heart diseases. 
No Epilepsy. 
Personal History:
Diet: Mixed
Appetite:Normal
Bowels:Regular
Allergies: Not allergic to any substances
Addictions: No. 
Menstrual History:
Not attained. 
Family History:
No significant family history. 
General Examination:
Pallor:Yes
Icterus:No
Cyanosis:No
Clubbing of fingers: No
Diagnosis:
Dimorphic Anaemia i.e iron deficiency and vit B12 deficiency. 
Questions from patient :
1.what type of food should be taken to improve blood value?
2.What are the risk factors for anemia?

After which he developed fever which is insidious in onset and gradually progressive, low grade, intermittent, associated with chills and relieved by medication.  Fever is associated with cold and cough.  Cough was productive with whitish mucoid sputum,  no foul smell and no blood associated.  History of past illness: Patient has past illness of shortness of breath, chest pain and pain in abdomen.  No diabetes No TB No asthama No thyroid No epilepsy.  Personal history: Diet: Mixed Apettite: Normal Bowel: r

This is an online eblog book discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio.                        CASE HISTORY Patient details: A 60 yrs old female resident of Nalgonda came to hospital.  Chief complaints: Dry cough: since 2 days Shortness of breath: since 1 day Chest pain :since 1 day Fever since :since 1 day Decrease in urine output : since 1 day.  History of present illness: Patient was apparently asymptomatic 2 days back.  Then she started complaining of dry cough since 2 days associated with chest pain.  She also complained shortness of breath since 1 day which is insidious onset and gradually progressive in naturenature and It is Grade - 3 i.e shortness of breath on doing less than ordinary .  She also complained of decrease in urine out put i.e oliguria since 1 day.  History of past illness: No diabetes, BP, Asthama, Seizures, TB or any
